Each promises a number that's more meaningful than the years since you were born. Some rest on solid research; others are marketing wrapped around a BMI reading. What biological age means
Biological age attempts to answer a real scientific question: how worn is this body compared with the typical body of the same chronological age? Two 50-year-olds can differ enormously in cardiovascular health, muscle mass, and cellular condition. Biological age tries to place you on that spectrum โ a 50-year-old with the physiology of an average 42-year-old would have a biological age of 42.
The research field is genuine and active. The most credible measures come from epigenetic clocks โ laboratory tests (Horvath clock, GrimAge, PhenoAge and successors) that read DNA methylation patterns, chemical marks on your genome that shift predictably with age. In population studies, people whose epigenetic age runs ahead of their chronological age show higher rates of age-related disease. These require a lab sample, not a web form.
What online biological age calculators actually do
A free calculator has no access to your DNA. What it typically does is collect self-reported inputs โ age, height, weight, blood pressure, resting heart rate, smoking status, exercise frequency, diet, sleep, alcohol โ and run them through a risk model, then express the output as an "age." The result is essentially your risk profile translated into years, which can be a useful motivational framing. It isn't a measurement of your cells.

Heart age
Of the consumer-facing measures, heart age has the clearest lineage: it repackages validated cardiovascular risk scores into a single intuitive number. Told your 10-year heart disease risk is 12%, most people shrug; told your "heart age is 61 when you're 48," they book a check-up. Public health bodies adopted it precisely because the framing changes behaviour. Because it's driven by blood pressure, cholesterol, and smoking, it's also actionable โ those inputs can move.
Metabolic age
Metabolic age is the softest of the group. Body-composition scales estimate your basal metabolic rate from bioelectrical impedance, then compare it to average BMR by age. There's no industry standard: the same body can produce different metabolic ages on two brands of scale. Impedance readings also shift with hydration, recent meals, and exercise. Treat it as a rough trend line for yourself over time, not a diagnosis โ and not comparable with anyone else's number.
Can you lower your biological age?
Partly, and the honest answer is more modest than the supplement industry suggests. Evidence consistently supports a handful of unglamorous factors: not smoking, regular physical activity (both cardio and resistance), maintaining a healthy weight, controlling blood pressure and blood glucose, adequate sleep, limited alcohol, and strong social connection. Small trials have shown modest reversals in epigenetic age markers with intensive lifestyle programmes, but results are early and not yet a basis for personal predictions.
What no intervention changes: your chronological age. It advances at one year per year regardless โ which is precisely why it remains the number on every legal document.
How to use these numbers well
- Track your own trend, not other people's numbers. The same calculator over time is far more informative than one reading.
- Check what's under the hood. A calculator that asks for blood pressure and cholesterol is modelling something real. One that asks only your age and weight is mostly restating BMI.
- Treat a bad result as information. The value of heart age is that it names a modifiable risk early.
- Don't pay for precision that doesn't exist. Consumer epigenetic tests still show meaningful test-retest variation โ the same sample can yield different ages.
